Present:  Larry Gibbs, Wayne Bockenstedt, Randy Keehner

Motion by Bockenstedt, seconded by Keehner to adopt the following designated physician for workman compensation claims for Clayton County workers.  Ayes:  Gibbs, Bockenstedt, Keehner

Designated Physician Policy Statement

Effective upon adoption of the Safety Program by the Clayton County Board of Supervisors, the following policy will be in effect regarding workers’ compensation illnesses or injuries.

Clayton County has designated the following clinics and locations as its workers’ compensation authorized treating clinics as provided by law under Chapter 85.39 of the Code of Iowa.  Employees with a non-emergency work-related illness or injury are required to have their initial evaluation with one of the listed clinics.  In the event that an employee needs immediate emergency care, seek the closest emergency care provider for the initial visit and assessment.  After which time all follow up visits will take place with a listed designated care provider.  If appropriate, for complex cases, employee can be referred to Tri-State Occupational Health for a second opinion or management of their case.

If an employee decides to go to another provider without the referral from the authorized treating clinic the employee will be responsible for all expenses related to those visits.  No workers’ compensation benefits may be claimed unless seen by one of the authorized clinics.

Motion by Keehner, seconded by Bockenstedt, to sign the Substance Abuse FY08 Contract with the state Department of Public Health.  Ayes:  Gibbs, Bockenstedt, Keehner
